If your child has an annual pass to Universal please let us know at the time of registration so we can adjust the cost for you.  Your child must have a current North Jax Medical Waiver form on file in order to participate in this event.  If you have not submitted one for 2025, please complete the form attached to your confirmation email.  Be sure to complete the entire section for Insurance information and have two adult witness signatures at the bottom of the form.
