Your child must have a current North Jax Medical Waiver form on file in order to participate in this event.  If you have not submitted one for 2024, please complete the form attached to your confirmation email.     Be sure to complete the entire section for Insurance information and have two witness signatures at the bottom of the form.

If you need to have the funds pulled from your child's account please call x320 or email annelisas@njbc.org to transfer the money.
