We will need a current NJBC Medical Waiver form must be submitted in order to participate. You may sign all forms and email back to annelisas@njbc.org, or drop your completed forms off in the black box located in the Kids wing. Hard copies will be available at the Kids Welcome Desk.
Helpful hints: Be sure to complete the entire NJBC Medical Waiver form with your insurance information and have two witness signatures at the bottom portion of the form.
